
A coordinated plan-of-care with the resident’s hospice service
Each resident will be receiving services from the hospice agency of their choice. The Hansen House will serve as the residence where hospice services are delivered. Our 24 hour staff will provide the hands on personal care needed, while the hospice agency will provide the medical Plan of Care.
A private room in a residential home
Each resident and family is offered the chance to personalize their room with small items and pictures from home that make the environment more personal and comfortable for the resident.
Open visiting hours
Family members and friends are encouraged to visit their loved one in our home. Visiting hours are open 24 hours a day, 7 days a week.
24-hour caregiver
Our caregivers are Arizona state certified caregivers with special training in end of life care. Our caregivers help each resident with their day to day care needs while assisting with comfort measures at the end of life. Families are also encouraged to assist with the care of their loved one, when they feel comfortable.
Comfort medication administration
Medications that are prescribed by the hospice agency will be administered by our staff, according to the resident’s Plan of Care. The resident’s elected hospice agency will be contacted by our staff for continued medical management.
Meal prep and comfort feeding
Some people don’t feel like eating much at the end of life, and others may want a particular food. Meals are prepared according to the patient’s personal preferences and their ability to eat. Staff will also follow the hospice agencies Plan of Care recommendations on feeding.
Bathing
Bathing will be provided as needed for comfort and according to the hospice agencies Plan of Care.
Dressing
Dressing is based on the resident’s preferences and the hospice agencies Plan of Care.
Incontinence care
The resident will remain clean and dry following directives from the hospice agencies Plan of Care.
Laundry/Linen services
Laundry services will be provided in the home and residents will have their items individually marked for personal and security purposes. Linen will be provided for each resident to include blankets, sheets, incontinence pads, pillows/pillow cases, towels and wash clothes. All will be laundered by The Hansen House staff.
Transfers and Turning
According to the hospice agencies Plan of Care for the resident, trained staff can assist resident with transfers to wheelchairs and assistance ambulating. Residents will be turned for comfort and to prevent skin breakdown when the resident is no longer able to turn themselves.
Individualized activities
Each resident is different and they have different activities they enjoy doing. On admission, individual interests are discussed to provide personalized care plans in our home. Activities include working puzzles, listening to music, drawing/coloring, board games, card games, watching favorite TV shows or movies and any other hobbies the resident may enjoy.
Aroma therapy
The Hansen House has a diffuser for aromatherapy treatments with a diverse collection of essential oils to chose from to aid in the comfort of the resident and their family.
Music therapy
Every resident is different. Upon admission to The Hansen House, we will assess the music styles and preferences of the resident. This music will be available for the resident and families enjoyment.
Life review and personal remembrance projects
The Hansen House will assist the resident and their families in life reviews at the bedside. There are also various remembrance projects that can be done to help families memorialize the life of their loved one.
Family support, along with the hospice agencies
Families are just as important to The Hansen House as our resident. The Hansen House along with the resident’s elected hospice agency, will provide family support during and after the resident has passed away.
